Accessing your medical record online
Your medical records hold information about you and your medical history. This includes information like any conditions or allergies you have and any medicine you’re taking.
They will also include most records from other NHS service you use, like the hospital, dentist or opticians, and will include letters, test results and appointment notes.
How to access your medical records online
You can access your medical records online via the NHS App. After creating an account and linking yourself to the surgery, you will gain access to your prospective record, which includes information from your date of registration onwards. For further access, please contact us as you will need to complete a form in person along with proving your identity, which will be assessed by your GP.
Proxy access
Proxy access forms can be found at reception and must be completed in person with both parties present, unless under exceptional circumstances. For proxy access to childrens records, this access is automatically disabled at age 11, pending a capacity assessment.
